I have received a copy of the Volunteer Handbook with the volunteer policies and procedures of Literacy Services of Wisconsin, Inc. ("LSW"). I have read and I understand its contents. I acknowledge that it is my responsibility to ask questions about anything I do not understand.

I agree to abide by the policies and procedures in this handbook as a volunteer for LSW, as well as rules and regulations that LSW may otherwise establish in its sole discretion. I also understand that LSW may interpret, modify, amend, or change the contents of this handbook at any time, with or without notice.

I further understand and acknowledge that this handbook provides guidelines and information, but it is not, nor is it intended to constitute, an employment contract of any kind. I understand that my volunteer service can be terminated at the option of either LSW or me.
